Death Certificate Information

Death records are available at the town/city hall in the municipality where the person passed away, at the town/city hall of the place of residence of the deceased, and in Augusta at the Bureau of Vital Statistics.

Certified copies of death certificates can be issued to anyone who has a legitimate interest in the decedent.  The fee is $15, for the first copy, and $6 for each additional copy.

Death records can be requested by mail by providing an application, positive identification including, but not limited to, a driver's license, passport or other government issued picture identification.  Should you not have direct and legitimate interest in the death record, you must complete the Release of Vital Records Application.  Send your application and identification along with a check payable to the City of Waterville and a self addressed stamped envelope to:
